παρανοϊκός is built on παράνοια, the ancient Greek word for a disordered or distorted mind, joined here to παρά (beside, beyond) and νόος/νοῦς (mind). The adjective suffix -ικός turns the noun into a description of a trait or condition, the same pattern seen in words like ελληνικός. Modern psychiatry borrowed παράνοια directly into English and other languages as a clinical term, and Greek later formed παρανοϊκός to match the international adjective paranoid.
