Ακραίος derives from άκρο, meaning edge or extremity, joined with the adjective-forming suffix -αίος. The underlying ancient Greek word ἄκρος meant topmost or outermost. Over time the meaning shifted from a physical edge to anything pushed to its furthest, most intense point.
