The verb comes from the Semitic root ء-ذ-ي, which carries the sense of harm or injury. It is built on the Form V pattern تَفَعَّلَ, which typically turns a Form IV causative verb into one meaning to undergo or experience that action on oneself. So while آذَى means to harm someone, تَأَذَّى means to have that harm happen to oneself, that is, to be hurt.
