Etymology
تَقَرَّر comes from the Semitic root ق-ر-ر, which carries the sense of firmness and settling in place. Form II, قَرَّرَ, turns that sense into an action: to make something firm, that is, to decide it. تَقَرَّر is Form V, the reflexive or passive counterpart, so it means a matter has become settled or fixed by decision, often with no named actor. This impersonal shape is why the word shows up so often in news reports and official notices.
